Is Nail Surgery Painful? Debunking Common Myths with Your Podiatrist

In this article:

If you’re dealing with an ingrown toenail, you’re not alone—and relief might be simpler than you think. Many people are surprised to learn that nail surgery is a straightforward, low-pain procedure designed to quickly resolve discomfort and prevent it from coming back.

At our podiatry clinics, we often see patients who’ve lived with discomfort for months (or years) simply because they were afraid of the procedure. In this article, we’ll debunk the most common myths around ingrown toenail surgery and explain what the process feels like—based on real patient experiences and expert insights.

What Is Ingrown Toenail Surgery?

Ingrown toenail surgery is a straightforward and effective procedure that removes part—or in some cases, all—of a toenail that’s growing into the skin. This often affects the big toe and can cause pain, swelling, redness, and even infection.

Podiatrists perform nail surgery using sterile tools and local anaesthetic to eliminate the source of the problem and prevent it from recurring. Most surgeries fall into one of two categories:

  • Partial Nail Avulsion (PNA): Only the side of the nail that’s causing the issue is removed. This is the most common and least invasive option.
  • Total Nail Avulsion (TNA): The entire nail is removed. This is usually reserved for severe or chronic cases.

Both procedures can include a treatment to the nail bed with a chemical (typically phenol) to stop regrowth of the ingrown portion.

Myth #1: Nail Surgery Is Incredibly Painful

Truth: With local anaesthetic, the surgery itself is painless.

Before the procedure begins, your podiatrist will numb the toe with a quick injection. This might sting briefly—similar to a flu jab—but once numb, you’ll feel no pain, just some pressure or gentle movement. The entire surgery usually takes approximately one hour, and you can walk out of the clinic the same day.

Myth #2: Recovery Is Long and Miserable

Truth: Most people will be walking with minimal pain after 48 hours.

After the procedure, the anaesthetic wears off over a few hours. Some patients experience mild tenderness or throbbing in the toe for 24–48 hours, which is easily managed with over-the-counter painkillers like paracetamol or ibuprofen. No intense exercise is recommended for 30-40 days to allow for full recovery.

Your podiatrist will provide aftercare instructions, including how to clean and dress the toe. You’ll also be advised to avoid intense physical activity for 1–2 weeks—but walking, working at a desk, or light movement is usually fine the next day.

Myth #3: You’ll Be Left with a Strange-Looking Nail

Truth: In most cases, the cosmetic results are excellent.

If only part of the nail is removed, the nail continues to grow normally—just slightly narrower. When phenol is used, the troublesome section won’t grow back, reducing the chance of future problems. In full nail removals, the area heals flat and smooth, and some patients find this more comfortable long-term.

Myth #4: Ingrown Toenails Are a Result of Poor Hygiene

Truth: Ingrown toenails can affect anyone—even with perfect hygiene.

While proper foot care can help reduce the risk, many other factors contribute to ingrown toenails:

  • Genetics (naturally curved or wide toenails)
  • Cutting toenails too short or rounded
  • Tight footwear
  • Repeated pressure from sports or long walks
  • Nail trauma or injuries

Why You Shouldn’t Delay Treatment

Ignoring an ingrown toenail may lead to:

  • Persistent or worsening pain
  • Swelling and redness
  • Infections that require antibiotics
  • Formation of granulation tissue (overgrown skin)
  • Trouble walking, working, or enjoying sports

If saltwater soaks, open shoes, or at-home remedies aren’t working, professional treatment is the safest and most effective next step.
